Scientific Names: Codeine and promethazine mixture.

Generic Names: Codeine is an opiate.

Promethazine is an antihistamine/sedative.

Desired Effects:

  • The desired effects of codeine include calmness, reduced anxiety, relaxation and pain relief.
  • On its own promethazine it is a mild sedative/depressant, although at higher doses has been linked to hallucinations. Promethazine is thought to increase the effects of opioids like codeine.
  • The effects of Lean will depend on the ingredients it is made with, but for Lean made from codeine and promethazine, the initial effects include mild stimulation before a mix of relaxation and euphoria. At higher doses, mild hallucinations and “out of body” effects like ketamine have been reported.

Side Effects:

  • Drowsiness
  • Confusion
  • Disorientation
  • Panic
  • Nausea & sickness
  • Itching and flushing of the skin
  • Constipation.
Poisoning or fatal overdose is possible with codeine, promethazine, and paracetamol individually, and combining them puts you at risk of death or permanent damage.

  • Start with a small dose.
  • If you are making it yourself, check the contents for paracetamol. Paracetamol overdose can not only kill but also causes permanent liver damage.
  • Adding alcohol (either drinking alcohol when drinking Lean, or if alcohol is an ingredient of the syrups being used) increases the risk of overdose.
  • Don’t drive under the influence of Lean.
  • Adding benzodiazepines (like diazepam or Xanax/alprazolam) to the mix greatly increases the risk of overdose.
  • Using nitrous oxide at the same time as Lean can make the effects of promethazine stronger and send you into a state of frenzy (called delirium).
  • Look after your mates.
  • If someone overdoses, call for an ambulance.
  • If you have naloxone use it as it can reverse the effects of opioid overdose and saves lives.

Long term risks:

This depends on what is in the Lean mixture. But with codeine-based mixtures, physical and psychological dependence with repeated use.

Short term risks:

Tolerance, accidents, overdose. Many UK codeine preparations contain analgesics such as paracetamol, which in high doses can be toxic to the liver and can kill.

Promethazine originally prescribed as an antihistamine for the treatment of allergic conditions but is now primarily used as a sedative and/or an anti-emetic.

In the United States codeine is available in pharmacy syrup preparations that also contain promethazine. The syrup is then mixed with a fizzy drink and sometimes a sweet to cover up the taste of the medicine to make a purple cocktail. Codeine and promethazine syrups are not available in the UK, so illicit preparations of Lean are made from mixtures that may include codeine and promethazine. However, the formulation used in the UK varies considerably and could contain just about anything. Some versions are sold that claim to contain to have other drugs such as THC or Xanax.
‘Lean’ is sipped or drunk as a liquid.
Both codeine and promethazine have medical uses, but the syrup mixture containing them both is not available in the UK.
The U.S syrup mixture of codeine and promethazine has been sold in the UK, but these are often fake products. UK versions of Lean are usually home-made and may contain a variety of ingrediants.
